Dental Plan Option 1

Covers includes;

• regular 6 monthly check ups,

• 2 hygienist visits each year

• All required dental x-rays *1

• Emergency treatment *2

• 10% discount on all dental treatment carried out in the practice*3

Costs;

Individual rate: £19 a calendar month, payable by standing order.

Couples rate: for 2 adults £36 a calendar month

Family rate*4: add £5 per child up to 12 years old

£8 per child 13-16 years old £10 per child in full time education living with the parents.

Notes*.

1. All required x-rays that can be taken at our practice- this is all routine dental x-ray examinations but excludes CT Scans and special x-ray examinations that may be necessary for referred specialist care.

2. Dental emergencies treated within the practice are covered as long as you have attended regularly and had any necessary treatment carried out. In situations where treatment has been ignored, or regular attendance has lapsed, there may be a charge for emergency care.

3. The discount will apply to all dental treatment including cosmetic dentistry and dental implants. It will not apply to any dental care requiring referral to specialists elsewhere.

4. Children are required to attend for their appointments at the same time as one of the adults named in their plan.

Conditions.

There is no requirement for you to be dentally fit to join this plan. It is a plan designed to spread the basic costs over the year, and is not a dental insurance scheme.

Your commitment is for 1 year at a time- renewable.

The cost will not increase during the year, but may be increased at the end of your year if it becomes necessary to increase dental fees.
