FREE SECOND OPINIONS

1st Impressions will provide a second opinion for dental implants and cosmetic dentistry at no charge if:

- you provide a treatment plan from another dental practice that has been provided in the previous 4 months.

- you provide a copy of satisfactory x-rays taken for that treatment plan.

- you request the free consultation at the time you make the appointment.

Information to assist you in obtaining the above documents:

All dentists (including NHS and specialists) are required to provide a written treatment plan after each dental examination, giving the treatment recommendations, the costs, the estimated timescale and the alternatives.

You are entitled to a copy of your dental records and x-rays if you wish.

The dentist may make a limited charge for these to be provided (but many will not make a charge- especially if they are stored digitally and can be e-mailed to you). If you don't have access to e-mail you can request that they be sent to our e-mail and we can print a copy for you.  info@1st-impressions.org

Dentists are required to supply them in a reasonable time frame and not delay the process.

Section 7 of the Data Protection Act (1998, gives the rights of access to individuals in respect of personal data held about them. A dentist is obliged to disclose an individual's dental records provided he/she has received: the individual's written request, the current maximum chargeable fee of £10 (£50 for manually stored records) and, if necessary, information required to confirm the authority of the person making the request, if not the individual. Disclosure must take place as quickly as possible or in any event within 40 days of receipt of the request.

When a dentist leaves a practice or retires from the profession they are required to leave the records with the practice.
